How do I become a client and begin referring patients to Northwest Medical Analytic Laboratory?

Contact our dedicated Client Service Representative by calling (360) 425-5620 or send an email to support@lcpath.com. Our team will provide you with all necessary supplies and requisitions and arrange for courier service to pick up specimens and deliver results to your office.

Where do I send patients for specimen collection?

We operate several conveniently located patient service centers in Longview and Salmon Creek, Washington. View a complete list of our service centers for maps, directions, and business hours.

Who can I speak with when I need to discuss a patient’s test results?

Northwest Medical Analytic Laboratory’s technologists and pathologists are available to discuss patient lab results when needed. Healthcare providers can call (360) 425-7915 to speak with a technologist or pathologist.

 

Billing Questions

Does Northwest Medical Analytic Laboratory bill my patients’ insurance companies?

NMAL bills most primary insurance companies if the information is provided when tests are ordered.

What information is needed to bill a patient’s insurance company?

Please provide the name of the insurance company, billing address, subscriber ID, group number, employer, and primary policy holder’s name.

Where can I obtain an ABN form (Advance Beneficiary Notice of Noncoverage) to use when a test may not be covered by Medicare?

Yes.
